The Core Tool: How a Cannula Removes Fat

A cannula is the instrument I use most often during liposuction. It has a handle that I hold and a tube that enters the body to remove fat. Cannulas come in slightly different thicknesses and lengths depending on the target area and the degree of reach required.

For example, two cannulas may have the same length—30 cm—but different diameters, such as 3 mm and 4 mm. While the differences seem small, that variation can matter during sculpting, because different body regions require different handling and efficiency. Longer cannulas are frequently used for areas like the thighs, where reaching from discreet incision sites down toward the knee can be essential.

Mechanically, the concept is straightforward: the cannula enters into the fat layer, moves back and forth through the fat, and suction pulls the loosened fat out through an opening at the tip. Through the front armpit incision, I can insert the cannula and suction strategically around the arm, including the elbow area where fullness can be noticeable. The second incision on the back side expands the range and allows contouring that can extend farther—often reaching toward the back and bra-line area when needed. Using these two access points helps shape the arm more evenly while keeping incision sites tucked away.

Abdomen Liposuction: Three-Direction Approach for Smoother Results

When contouring the abdomen, I use incision placement to make the work both effective and even. Common incision sites include one inside the navel and another along the hairline in the pubic area. In practice, these incisions are made smaller than people expect, because they should not be easily noticeable.

For lower abdomen fat removal, I often approach from multiple directions. That means suctioning through the pubic hairline incision, suctioning through an additional lower access point as needed, and then also working through the navel incision. By sculpting from three directions, it becomes easier to create uniform thickness and reduce the risk of surface irregularities. The abdomen is an area where “evenness” is everything—good contour depends on consistent removal rather than simply removing the most fat.

Flank Liposuction: Shaping the Waistline With Smart Entry Points

The flanks play a major role in how the waistline looks from the front and side. For flank contouring, access can be gained through the navel incision or through an inguinal (groin) incision. These locations allow the cannula to reach the targeted fat while keeping scars in areas that are naturally concealed by anatomy or clothing.

This area is also where technique matters: the goal is not just to reduce bulk, but to create a smooth transition from the abdomen into the side body so the waistline appears refined rather than abruptly “carved.”

Love Handle Liposuction: Tailbone or Dual-Side Incisions

For love handles, I may use one incision or two depending on the shaping plan and what will be most discreet. To hide an incision well, one option is placing it between the buttocks near the tailbone. Another approach is to use two incisions, one on each side, which can sometimes allow more symmetrical access and control depending on the anatomy and the contour goals.

The best choice depends on the patient’s shape and where the cannula needs to travel to create a balanced silhouette. The guiding principle remains the same: optimize the sculpting pathway while keeping the incision location as hidden as possible.

Thigh Liposuction: Reaching the Knee With the Right Cannula Length

Thigh liposuction is a prime example of why tool selection and incision planning are inseparable. In many patients, about 70 to 80% of thigh fat is concentrated on the inner and outer thighs. Because of that distribution, the most important contouring work typically focuses on the inner and outer thigh to create a cleaner line and more attractive proportion.

For the thighs, I typically make incisions both in front and in back. One incision can be hidden in the gluteal fold, and another can be placed within the hairline area. However, incision location affects how far the cannula can reach. If the incision is positioned such that reaching the knee is difficult, a longer cannula becomes necessary.

This is where a 40 cm cannula can make a major difference. Inserted through the appropriate incision site, a longer cannula can reach down to the knee area, allowing consistent contouring along the length of the thigh rather than leaving a transition zone. For the back-thigh incision, sometimes a long cannula is used and sometimes it isn’t—but because that incision can be closer to certain target zones, many cases can still reach the knee with a 30 cm cannula.

To reduce outer-thigh “saddlebags,” I commonly suction from the outside using both the back-thigh incision and the front incision. This dual-direction approach supports smoother blending and a more natural contour.

Anesthesia and Step-by-Step Flow: Sedation, Tumescent, Sculpting

During the procedure, sedation is used, and the depth of sleep is adjusted so the patient doesn’t wake up and so movement—especially in areas like the arms—remains minimal. The intention is for the patient not to wake in the middle of the procedure.

After sedation is administered safely, the next key step is injecting tumescent solution evenly into the fat planned for removal. This solution helps firm the tissue and swell it slightly, preparing the area for more controlled suction. Finally, the cannula is used to remove fat carefully and evenly according to the sculpting plan.
